And many assume that a workflow model that comes with a tool is a good
one to follow without looking at what their processes actually are and
whether they fit.

That's right, the simpler the better (although I would probably add a
Review -> Rework loop in as an option between Author and Approve).

I couldn't agree more - in fact that's the basic thrust of my
OpenPublish talk :)
